DAG04: Need Assessment and Individual Care Plan (ICP)
Aims: This course is designed to provide participants with opportunities to develop knowledge and skills on geriatric assessment and designing individual care plans. This will be achieved by introducing the purposes of comprehensive geriatric assessment in physical, psychosocial, family, network, lifestyle, nutritional, environmental, mental, functional and cultural assessment. Learning objectives At the end of the module, the participants will be able to:
1. To understand the concepts and theories of geriatric assessment
2. To understand how to make use of various type of geriatric assessments in developing ICP
3. To familiarize with forms and procedures to be used in designing individual care planning
4. To introduce Residential Assessment Instrument (RAI) and long term care assessment
5. To discuss care planning, quality assurance and outcome measures
Teaching methods: Classroom teaching, case study and group discussion

Medium of instruction: The course will be taught in Cantonese and supplemented with English. All teaching notes will be in English orChinese
Assessment: The assessment of this course comprised of two pieces: 1. examination/test of assessment toools (50%) 2. course work for drafting ICP and attendance (50%) Participants must have 80% attendance to be eligible for the assessment Awards certificates: A certificate will be awarded to participants who have attended at least 80% of the total training hours and satisfactorily completed the assessment. Participant who have completed 3 basic courses and 7 advanced courses will be awarded "Diploma in Applied Gerontology"

A completed application form must be accompanied with the following documents: photocopies of degree/diploma certificates and testimonials or other relevant evidence of the applicant’s working experience. Exemptions may be considered on an individual basis.
